Responsibility:
1.
To follow the Quality Policy &
Contamination prevention policy of the company.
2.
Ensure that there is no
contamination, while doing the process.
3.
Participate in Process Risk
Assessment Sessions to understand the process & other hazards involved in
product related activities.
4.
Create awareness among all
employees from relevant functions on process hazards, emergency scenarios
before start-up of product.
5.
Lead the Pre-Startup-Safety Review
for safe start-up of product.
6.
Analyses the PSSR non-compliance
data during the product startup over a period of time.
7.
Participate in MOC sessions to
understand the risk involved in the change.
8.
Lead the Pre-Startup-Safety Review
related to all MOCs.
9.
Approve
permits
required in the area for safe execution of activities.
10.
Educate employees on important of
PPE from time to time through different platforms & methods (other than
formal training).
11.
Ensure overall PPE compliance as
per risk assessment.
12.
Ensure all the incidents are
reported within the time and follow-up with the production.
13.
Detailed
Investigation of all incidents to identify root cause and control measures.
14.
Analyses the incidents data in
regular intervals.
15.
Create awareness among all
employees on lessons learned from incidents.
16.
Analyzing the day to day alarms
generated from DCS/SIS/PLC to identify the issues.
17.
Recommend solution in consultation
with Head P&PS to prevent recurring alarms.
18.
Managing / conducting HSE training
for employees in the Blocks.
19.
Create awareness among all
employees on process safety, safety and other HSE systems.
20.
Suggest improvement in training
contents based on feedback from training sessions.
21.
Execution of mock drills – area
specific.
22.
Ensure healthy condition of all
emergency equipment.
23.
Identifying critical operations
and ensure all control measures are placed.
24.
Ensure all critical equipment
maintenance carried out as per schedule.
25.
Ensure all critical Instruments
maintenance carried out as per schedule.
26.
Summarize issues in shift handover
report and escalate further.
Authorities:
1.
Authorized to stop product /
operation for an operation started without PPE compliance. This will include
prohibiting any person from working in the plant without proper PPE.
2.
An unsafe act from an individual
or activity where unsafe condition is observed.
3.
Authorized to issue Violation
slips as per RISE.
